Two state addresses carry Utah Medicaid: prism.health.utah.gov and elt.medicaid.utah.gov. Both resolved in a Domain Name System lookup on 10 September 2026. Utah reads as fee-for-service with no dental benefit manager — no DentaQuest, no MCNA, no LIBERTY sitting between the practice and the programme — but that reading is unconfirmed, and this page is not going to pretend otherwise.
The useful thing about Utah is what we will not claim
Most of these state pages exist because there is a trap: a carve-out nobody expects, a county split, a dead bookmark, a benefit manager that changed hands in the middle of a contract year. Utah has none of that in front of us. What it has instead is an unsettled fact, and the honest handling of an unsettled fact is to name it.
So: do not assume there is no benefit manager, and do not assume there is one. The check was a Domain Name System lookup on 10 September 2026 and nothing more. prism.health.utah.gov resolved through an Amazon load-balancer name to two addresses (44.237.249.245 and 184.33.219.187). elt.medicaid.utah.gov returned a single address (168.180.165.83). We did not sign in, and we make no claim about whether either was reachable from any particular network — government portals routinely refuse traffic that does not come from an ordinary browser, so a failure from elsewhere proves nothing about the portal.
Which of the two addresses answers which question is not established here either. Ask when you call.
What to do
- Bookmark both addresses and reach them from the bookmark rather than a search result.
- Ring Utah Medicaid and confirm who adjudicates the dental claim before you build an estimate on the assumption that the state does.
