The portal is forwardhealth.wi.gov, which serves both Wisconsin Medicaid and BadgerCare Plus. For most of the state that is the whole answer: dental is fee-for-service, and you verify and bill ForwardHealth directly with no benefit manager in between. Five counties are the exception, and knowing which five is the difference between a paid claim and a denial nobody can explain.
The five-county exception
Dental runs fee-for-service through ForwardHealth everywhere in Wisconsin except Milwaukee, Ozaukee, Racine, Washington and Waukesha. In those five south-eastern counties the dental benefit runs through an HMO instead, and the second portal below is the one that matters.
That is a geographic rule, not a plan rule, which makes it easy to get wrong in a practice that draws patients across a county line. An office in Waukesha treating a patient from Jefferson County and an office in Jefferson treating a patient from Waukesha are answering two different questions with the same portal open.

Both hostnames resolved in a Domain Name System lookup on 10 September 2026. forwardhealth.wi.gov answers as an alias for www.forwardhealth.wi.gov, and pwp.skygenusasystems.com as an alias for sgusa.skygenusasystems.com — normal for portals of this kind, and worth knowing only because a bookmark saved against one of those inner names can break independently of the one you were given.
That DNS lookup is what we verified and the limit of it. We did not attempt to sign in. Several state portals refuse requests that do not look like an ordinary browser, so a failed fetch from outside a practice network proves nothing about whether the portal is up.
A lookalike domain is live, and it turns up in search
A domain named forwardhealthproviderportal.com was registered on 23 May 2026 at 13:41 UTC and was still resolving when we re-checked on 10 September 2026. Wisconsin's is the one we would flag hardest, because unlike most of the family it appears in ordinary search results for Wisconsin rather than only being reachable if you type it.
It is one of seven domains built to the same pattern — a real portal's name with providerportal.com appended — all registered through the same registrar under privacy protection, all behind Cloudflare, six sharing the same pair of nameservers, and two of them created two and a half minutes apart. We read the page titles and nothing further, so we make no claim about what the sites do.
The claim we do make is narrow: it is not ForwardHealth. The address that is ForwardHealth ends in .wi.gov.
What to do
- Bookmark forwardhealth.wi.gov and reach it from the bookmark rather than from a search result.
- Check the patient's county before the plan. If it is Milwaukee, Ozaukee, Racine, Washington or Waukesha, the dental benefit is with the HMO, not fee-for-service.
- Record the county rule in the practice management system rather than in someone's memory — it is exactly the kind of rule that leaves with a staff member.
- If a Wisconsin claim denies for no visible reason, confirm which of the two routes the patient's county puts them on before appealing.
