Until what age does insurance cover a space maintainer?
Federal plans cluster at under age 19 and one runs to 22. Commercial caps run lower, from through age 13 to under 19, and are often once per space per lifetime.
Space maintainers (D1510 and the fixed and removable variants D1516, D1517, D1520, D1526 and D1527) carry an age limit on almost every plan that covers them, and the federal plans in this evidence cluster tightly at under age 19 — covered through 18 — with one exception that runs to 22. Commercial plans run lower and vary more: one Delta company caps at through 13, another at up to age 14, UnitedHealthcare's commercial rule is under 16, and Blue Cross Blue Shield of Arizona publishes through 15. As of September 2026 Kaylie publishes no space-maintainer denial measurement, so the numbers below cover age denials across every code.

What the carriers' own policies say
Four federal plans, four ways of writing one boundary. The MetLife Federal Dental Plan, 2026 FEDVIP brochure:
"D1510 Space maintainer - fixed, unilateral – per quadrant - Limited to children under age 19"
The UnitedHealthcare FEDVIP dental plan, 2026 OPM brochure:
"D1510 Space maintainer - fixed, unilateral – per quadrant - Limited to covered persons under the age of 19 years, once per consecutive 60 months."
United Concordia's 2026 FEDVIP dental brochure:
"D1510 Space maintainer - fixed, unilateral – per quadrant – Limited to one per three years for members under age 19"
The Delta Dental FEDVIP 2026 brochure: "D1510 Space maintainer - fixed, unilateral – per quadrant – For dependent children under age 19".
All four mean covered through 18, and two of them state a replacement interval: 60 months at UnitedHealthcare, three years at United Concordia. BCBS FEP Dental's 2026 brochure is the outlier on age:
"D1510 Space maintainer - fixed, unilateral – per quadrant - Limited to individuals up to age 22"
"Up to age 22" is the phrase that cannot be resolved from the words. Read against that program's family rule of children under 22, it points at through 21 — but that is an inference from a different sentence, and it is worth confirming with the payer.
The same wording carries across the whole code family on the four plans that repeat it, so the fixed unilateral appliance and the bilateral and removable variants share one age.
Why it depends on the plan
Commercial caps sit lower than the federal ones and spread wider. UnitedHealthcare's carrier-wide rule is one per 60 months for covered persons under 19 on its federal plan and under 16 on commercial. Delta Dental of Minnesota publishes one per lifetime through age 13. Delta Dental of Tennessee publishes members up to age 14. Delta Dental Northeast publishes through age 16. Southland Benefit Solutions prints "up to age 14". Blue Cross Blue Shield of Arizona publishes through age 15, and Blue Cross Blue Shield of Alabama through age 18. Ameritas publishes a range of through 13 to 18 depending on plan, and Anthem a range of through 14 to 18.
Three things other than the age decide whether the appliance pays. The replacement interval — one per 60 months, one per three years, or one per space per lifetime — which matters because a child who loses a maintainer needs a second one inside the window. The quadrant or space rule, since these codes are billed per quadrant or per space and a bilateral appliance is a different code from two unilaterals. And the material, in the rare case a plan writes it in: Blue Cross Blue Shield of Alabama's rule covers space maintainers not made of precious metals.
What to do
- Ask for the age and the replacement interval together. A second maintainer inside the interval is refused on frequency, not age, and reads the same way to a parent.
- Convert the phrase before recording it: "under age 19" and "under the age of 19 years" are through 18; "up to age 14" is unresolved and needs confirming.
- Check the whole code family, not just D1510. The fixed bilateral, removable and recementation codes may sit under one age or under different ones.
- Where a child is above the cap and the space still needs holding, quote the appliance as a patient charge before placement.
- Note the plan's answer in the patient's record with the date, because the second maintainer on the same child is usually the one that gets refused.
