Delta Dental PPO vs Premier: what's the difference?

Premier is Delta's traditional fee-for-service network and PPO the reduced one — same contract, two allowances, and on a PPO plan a dentist in both networks is paid at the PPO rate.

Delta Dental PPO and Delta Dental Premier are two networks sold under one participating provider agreement, and the difference between them is the size of the allowance. Premier is the older, traditional fee-for-service program; PPO is the reduced fee-for-service program. Each pays the lesser of the fee you submitted and that tier's allowance, and if your office is enrolled in both — which is what most participation agreements now select, though Premier-only participation exists — Delta pays a PPO patient's claim at the PPO rate. Delta states only that "Premier fees are typically higher than Delta Dental PPO fees"; no Delta document states the size of that gap, and neither do we. As of August 2026, one Delta company published six separate fee schedules to a single contracted office, pricing D0120 three different ways across them.

That is one state company, one office's contract, read in July and August 2026. The point is the shape, not the dollars: "your Delta fee schedule" is not one list. It is a grid of tier by provider type, and the same periodic exam is worth three different amounts inside it depending on which cell of the grid a claim lands in.

What Delta Dental's own documents say

On the two programs, from a provider handbook written for dentists:

"Dentists who participate in Delta Dental PPO agree to accept the lesser of their actual fee, filed fee, or the applicable Delta Dental PPO Table of Allowances fee for Delta Dental PPO covered patients as payment in full."

"Premier reimbursement is determined on the same basis… the fee the Premier dentist may charge and collect is limited to the lesser of your pre-filed fee; the fee you submit on the claim; or the Delta Dental Premier fee established for that location."

On what the patient gets at each tier, written for employers:

"Think of the three options — PPO, Premier and non–Delta Dental dentists — like a tier system… They can't charge PPO members more than that fee. Premier fees are typically higher than Delta Dental PPO fees, but PPO members still enjoy cost protection at Premier dentists."

The number a plan pays from has a name of its own, and Delta defines it for the employer buying the plan rather than for the office:

"Reimbursement is the amount that your plan bases its payment on, also known as the maximum plan allowance. This is separate from the percentage at which certain services are covered."

And the enrolment rule that decides which one applies to you. Reading across Delta's member companies' documents, if a dentist is in both PPO and Premier, a PPO patient's claim pays at the PPO rate; only a Premier-plan patient's claim pays from the Premier allowance. One member company states it from the signing side: to become and remain a Delta Dental PPO participating dentist you must "be a Delta Dental Premier® participating dentist" — Delta Dental of New Jersey 2023 Participating Dentist Handbook. Premier is not the alternative to PPO there; it is the door into it.

Why it depends on the plan

Premier is a tier of your contract. Whether a given patient's plan honours it is a purchasing decision the employer made:

  • On a plain PPO plan, a Premier dentist may be reimbursed at Premier fees or at PPO fees. Delta puts that choice to the employer directly: "How Premier dentists are reimbursed under your PPO plan is up to you."
  • A PPO patient may generally be treated by a Premier dentist and pays more for it. Delta describes the Premier network to employers as a safety net, where members "can expect the same perks of a high-quality network and easy access, but they pay a bit more out of pocket."
  • On a Delta Dental PPO MAC plan, the Premier allowance stops being the ceiling for payment purposes and the PPO fee applies to every tier.

So two patients holding cards that read "Delta Dental PPO" can pay your Premier office two different amounts, legitimately, because their employers bought different reimbursement bases. Nothing in the sources says where that basis is printed for the office to read.

"Delta Dental" is also not one insurer. It is a federation of separately incorporated state companies sharing a brand, a provider network and processing policies, each underwriting its own policies under its own state regulator — and in Texas, by Delta's own footer, "Delta Dental PPO provides a dental provider organization (DPO) plan."

What to do

  • Load both schedules, not one. If you are enrolled in PPO and Premier, your practice management system needs the PPO schedule and the Premier allowance separately, plus the hygienist and specialist variants if your office bills under them.
  • Expect to be paid the PPO rate on every Delta PPO claim while you are enrolled in both. Premier's allowance is the payment basis only when the patient's plan is Premier-only, or your office is.
  • Ask the payer two specific questions before quoting: which network the plan covers, and — if you are a Premier office — whether Premier dentists are reimbursed at Premier fees or at PPO fees under this plan.
  • Quote from the allowance, not the coverage percentage. A percentage applies to the tier's allowance, not to your fee.
  • Do not estimate the PPO-to-Premier difference from a rule of thumb. No carrier document states it, and the only place it exists is your own two contracted schedules.
