# Who actually administers this dental plan?

> Often not the company printed on the card. Delta Dental is 39 independent companies. Unum's dental is underwritten by Starmount Life. Colonial Life dental shares Unum's dental provider portal and its phone line. Aflac's network dental is administered by Aflac Benefit Solutions and SKYGEN USA. Meritain administers the employer's own plan.

Three different companies can stand behind one dental card: the one whose name is printed on
it, the one that answers the phone and holds the benefit detail, and the one the claim is
actually sent to. On most cards they are the same company and nobody has to think about it.
On the six carriers below — each read on its own public pages in September 2026 — they are
not, and the mismatch is where a verified claim still goes
wrong — misrouted, filed under a payer ID that does not exist in your clearinghouse, or paid
against a fee schedule nobody at your office has seen.

Kaylie has not measured how often dental claims are misrouted for this reason, and we will
not print a rate we did not measure. What follows is each carrier's own published statement of
who does what.

## Who answers, by carrier

| Name on the card | The entity behind it | Where an office checks | Source |
|---|---|---|---|
| Delta Dental | One of "39 independent Delta Dental companies"; the claim goes to the patient's company, not your local one | Dentist dashboard on deltadental.com — member ID, name and date of birth | [Delta Dental — Dentist FAQ](https://www.deltadental.com/dentist/dentist-faq/) |
| Unum (dental) | Underwritten by Starmount Life Insurance Company; Provident Life and Casualty in New York; self-insured dental claims administered by Starmount | Unum dental provider portal; (888) 400-9304 | [Unum — Dental insurance for employers](https://www.unum.com/employers/employee-benefits/dental-insurance) |
| Colonial Life (dental) | Colonial Life & Accident Insurance Company; named among the underwriters on the same dental provider portal as Unum | coloniallifedental.com; 1-888-400-9304 | [Colonial Life — Dental and vision insurance](https://www.coloniallife.com/employers/employee-benefits/dental-and-vision-insurance) |
| Aflac (network dental) | "Administrative services are provided by Aflac Benefit Solutions, Inc. and SKYGEN USA, LLC." | SKYGEN-hosted provider search; the number on the dental ID card | [Aflac — Dental provider search](https://www.aflac.com/about-aflac/providers/dental-provider-search.aspx) |
| Aflac (supplemental dental) | Aflac itself — "No network requirements"; pays a flat benefit amount; does not coordinate with other insurance | Benefits lookup by policy number plus procedure code; 1-800-992-3522 | [Aflac — Supplemental dental providers](https://www.aflac.com/about-aflac/providers/supplemental-dental-providers.aspx) |
| Meritain Health | The employer's own plan; Meritain administers it, and the terms are the group's | account.meritain.com; 1.800.566.9311 | [Meritain Health — Provider services](https://www.meritain.com/about-us-self-funded-employee-benefit-plans/meritain-health-provider-services/) |
| Companion Life | Companion Life Insurance Company; its "Dental by Design" plans, with no provider eligibility route published as of September 2026 | Directory at companionlife.go2dental.com; the number on the card | [Companion Life — Dental providers](https://www.companionlife.com/providers) |

Each row is what that carrier states on its own public pages, read in September 2026.

## The four shapes this takes

**One brand, many insurers.** Delta Dental is the clearest case: 39 companies, each with its
own payer ID and claims address, and a claim goes to the patient's company rather than to
whichever Delta Dental you signed with. The dentist dashboard is what resolves it. Detail in
[verifying eligibility with Delta Dental](/blog/delta-dental-eligibility-verification).

**One brand, a different legal insurer.** Unum's dental PPO plans are underwritten by
Starmount Life Insurance Company, so a remittance advice reading Starmount is correct and a
clearinghouse search for "Unum" may find nothing. Same pattern, different direction, at
Colonial Life, whose dental sits on the same provider portal and the same phone line as
Unum's, and whose dental forms are served from `forms.unum.com`.

**One brand, an outsourced administrator.** Aflac's network dental plans state plainly that
administrative services come from Aflac Benefit Solutions and SKYGEN USA, and the provider
directory is hosted accordingly. The office-facing systems are the administrator's, not the
brand's.

**One brand, and the plan is the employer's.** Meritain administers self-funded employer
plans. There is no Meritain benefit design to learn: the frequencies, waiting periods and
maximum belong to the group's plan document, which the portal will show you. Its
machine-readable file search returns a separate file per group ID, which is that fact drawn as
a picture.

## Why this costs money rather than just time

- **A claim filed to the wrong entity is not denied, it is unanswered.** It ages quietly and
the timely filing clock keeps running against you.
- **The payer ID you cannot find is usually filed under the other name.** Search the
underwriter as well as the brand — Starmount as well as Unum.
- **A remittance from a name nobody recognises gets posted to the wrong payer** or sits
unposted, and every collections report built on that is wrong about the same money.
- **A brand-level note in your practice management system is a false memory.** "Unum pays
80% on basic" is a sentence about one employer's plan. Record benefit facts against the
group or policy number, never against the carrier name.
- **The wrong product family means the wrong arithmetic.** A supplemental Aflac policy pays a
flat amount and does not coordinate with other insurance; a network dental plan pays a
share of an allowed amount. Same brand on the card.

## What to do

- Add two fields to how your office records insurance: **who adjudicates** (the entity, and
for Delta the member company) and **the payer ID and claims address that go with it**. Fill
them at verification, not at billing.
- File every benefit answer against the **group number or the policy number**, so the note is
true for the next patient on that plan and silent about patients on another.
- When a clearinghouse cannot find a payer, search the underwriting company before concluding
the claim must go on paper.
- Keep one payer record per money trail, not per brand name — a split record splits your
  ageing report.
- Ask the administrator question out loud on the verification call. It takes one sentence and
it settles where the claim goes.
