# How do I verify eligibility with Colonial Life dental?

> Colonial Life offers individual dental PPO and individual dental fee schedule policies and does not currently offer group dental. Verification runs through the dental provider portal and 1-888-400-9304. Payment goes to the policyholder unless the patient assigns it to you so the assignment on your form is doing real work.

Two facts about Colonial Life dental decide how you verify it, and neither is on the card.
The first is that these are **individual** policies bought by the person, not group coverage
bought by an employer: as of September 2026 Colonial Life's own employer page describes its
dental offering as
"individual dental PPO and individual dental fee schedule insurance" and says the company
does not currently offer group dental ([*Colonial Life — Dental and vision
insurance*](https://www.coloniallife.com/employers/employee-benefits/dental-and-vision-insurance),
2026). There is no group number to look up and no employer plan document behind it. The
policy is the plan.

The second is that there are two different products under that one name, and they pay by
different arithmetic. A dental PPO pays a share of an allowed amount. A dental **fee
schedule** policy pays a set amount per procedure. Asking "what's the coinsurance on a
crown" of a fee schedule policy gets you an answer that does not exist. Ask which of the two
you are holding, and for a fee schedule policy ask for the scheduled dollar amount for the
specific code.

Kaylie has not measured Colonial Life dental verification outcomes and will not print a
number we did not measure. What follows is what Colonial Life publishes.

## Where you check

The member and provider site is [coloniallifedental.com](https://www.coloniallifedental.com/),
which carries the dentist directory at
`coloniallife.go2dental.com` and publishes **1-888-400-9304** for support, Monday to Friday
8 a.m.–8 p.m. ET and Saturday 10 a.m.–4 p.m. ET. The underwriter named in the site's
disclosure is **Colonial Life & Accident Insurance Company, Columbia, SC**.

That phone number is the same one printed on the Unum dental provider portal, whose footer
names Colonial Life & Accident among its underwriters, and the individual dental PPO claim
form on coloniallifedental.com is served from `forms.unum.com`. If your office already has a login for the Unum dental
provider portal, ask whether it covers these patients too rather than opening a second
onboarding — see [verifying eligibility with Unum
dental](/blog/unum-dental-eligibility-verification).

## What Colonial Life says about claims and estimates

Three sentences from Colonial Life's own dental and vision page carry most of the operational
weight:

> "Most dentists will file insurance claims on behalf of their insured patient."

> "The benefit can be paid to the employee, but most of the time the employee will choose to
> have the benefits paid directly to the dentist."

> "the employee's dentist will be able to submit a pre-determination/pre-estimate to allow
> for an estimate of benefits available."

— [*Colonial Life — Dental and vision
insurance*](https://www.coloniallife.com/employers/employee-benefits/dental-and-vision-insurance),
2026

Read the second one twice. Colonial Life states that "claim payments will not default to the
provider" and that most dental offices' standard forms require assignment. On an individual
policy, the money is the policyholder's until they sign it over. An office that files without
a signed assignment of benefits can do everything else right and watch the payment land in
the patient's mailbox.

The third sentence is the one to use. Colonial Life names the pre-determination — a
pre-treatment estimate filed on the claim form before treatment — as the way a dentist gets
an estimate of benefits available. On a product where nobody can tell you a group's plan
document because there is no group, that is the reliable route for anything expensive.

## What commonly goes wrong

- **The office looks for a group number.** There is not one. Take the policy number.
- **A fee schedule policy is quoted as a percentage.** The estimate will be wrong by whatever
the gap is between the scheduled amount and your fee.
- **No assignment of benefits on file.** Colonial Life says payment does not default to the
provider. This is the single most expensive mistake on this carrier.
- **An out-of-network estimate built from the office's full fee.** If the plan pays against a
scheduled or maximum allowable amount rather than your fee, the patient's share is larger
than the software says — see [MAC versus UCR and why out-of-network estimates are
  wrong](/blog/dental-mac-vs-ucr-why-out-of-network-estimates-are-wrong).
- **Verifying once and treating the answer as durable.** An individual policy lapses when the
person stops paying, with no employer termination feed behind it.

## What to do

- Take the **policy number** and the **product name** at scheduling, and write both into the
  insurance record.
- Ask on the call which product it is — PPO or fee schedule — and record the answer where the
  next person will read it.
- Get the assignment of benefits signed before you file, every time.
- File a pre-determination for crowns, endodontics, perio and anything over your practice's
estimate threshold, and quote from what comes back.
- Confirm your directory listing at `coloniallife.go2dental.com` under the plan on the card,
  not under Colonial Life generally.
- Re-verify at each new course of treatment rather than each new benefit year, because the
policy's status is the patient's payment history, not an employer's enrolment.
