# Can I get a third cleaning if I'm pregnant or diabetic?

> On many plans yes, and the qualification is documentary: a physician statement, a diagnosis paired to the claim, a pocket-depth reading, or advance registration. Some plans price the extra cleaning at a copay instead of covering it. As of September 2026, 1.7% of MetLife adult cleaning lines Kaylie saw were refused for frequency. The plan document decides.

On many plans, yes — and the extra cleaning is published and named in the benefit document rather than something a practice negotiates. What decides it is paperwork, not the patient's word: the carriers that publish the rule gate it on a physician's written statement, a medical diagnosis carried on the claim, a measured pocket depth, or registration with the carrier before the visit. Two more things matter before it is offered. Some plans price the third cleaning at a copay instead of covering it, and where the extra cleanings sit inside the ordinary benefit they still spend the plan's annual maximum — so more frequency is not more money. As of September 2026, across the adult cleaning lines Kaylie measured, 1.7% at MetLife and 1.0% at Cigna were refused for frequency.

## What we measured

| Code | Carrier | Lines | Practices | Refused for frequency | Share |
|---|---|---|---|---|---|
| D1110 adult cleaning | MetLife | 5,178 | 27 | 87 | 1.7% |
| D1110 adult cleaning | United Healthcare | 1,734 | 24 | 30 | 1.7% |
| D1110 adult cleaning | Guardian | 3,266 | 24 | 51 | 1.6% |
| D1110 adult cleaning | Cigna | 4,754 | 26 | 48 | 1.0% |
| D1110 adult cleaning | Delta Dental | 16,803 | 27 | 120 | 0.7% |

Adjudicated claim lines across the practices Kaylie serves, all dates in the record, measured September 2026; a line is one procedure on one claim, and only cells with at least five practices and 100 lines appear. This is what we saw, not any carrier's rule — the rate at which an adult cleaning is refused for frequency at all.

## What the carriers' own policies say

The federal plans publish the rule and the gate together. Delta's federal brochure allows three and names the evidence: "D1110 Prophylaxis - adult – Limited to two in a calendar year (or three for a pregnant individual)", and separately, "Three prophylaxes for pregnant individuals are covered in a calendar year. A statement from the patient's physician documenting the patient's medical condition must be provided." ([*Delta Dental FEDVIP 2026 Federal Employees Dental Program Brochure*](https://www.opm.gov/healthcare-insurance/healthcare/plan-information/plans/pdf/2026/brochures/02AP-05.pdf).)

MetLife ties it to a diagnosis and to one option tier: "D1110 Prophylaxis – adult - Limited to 2 in 12 months; High Option only: a third cleaning is covered when paired with a diagnosis of pregnancy or diabetes at a limitation of 1 in 12 months" ([*MetLife Federal Dental Plan 2026 FEDVIP Brochure (OPM)*](https://www.opm.gov/healthcare-insurance/healthcare/plan-information/plans/pdf/2026/brochures/02AP-11.pdf)). United Concordia gates it on registering first: "D1110 Prophylaxis – adult – High Option - Limited to three per calendar year; one additional cleaning during pregnancy when registered in MyDentalBenefits." ([*OPM United Concordia FEDVIP Dental Brochure 2026, contract 02AP-14*](https://www.opm.gov/healthcare-insurance/healthcare/plan-information/plans/pdf/2026/brochures/02AP-14.pdf).)

The military programme publishes the widest list of qualifying conditions: "A third prophylaxis in a consecutive 12-month period is allowed for pregnant patients. … A third prophylaxis in a consecutive 12-month period is allowed for patients diagnosed with diabetes, coronary artery disease (heart), cerebral vascular disease (stroke), rheumatoid arthritis, lupus, oral cancer and recipients of an organ transplant." ([*TRICARE Dental Program Handbook Supplement – Benefits Booklet*](https://tricare.mil/-/media/Files/TRICARE/Publications/PubsNotOnPubsPage/TDP_Benefit_Supplement_Annual.pdf), January 2025.)

Two plans replace the diagnosis with something measurable, or waive the interval. One unlocks the third and fourth visits on a charting number: "Covered for additional prophylaxis or periodontal maintenance (up to four total treatments combined) if your gums have Pocket Depth readings of 5mm or greater" (*Individual & Family Washington Family Plan EOC 2025*). Another suspends the interval altogether: "If you are pregnant, you are eligible for a cleaning in the third trimester of pregnancy regardless of when you had a previous cleaning." ([*Delta Dental PPO 1000 Oregon Group Dental Plan Handbook 2026, Sample*](https://www.deltadentalor.com/-/media/DeltaDental/Downloads/Oregon/plans/2026/small-group/Sample-handbooks/Delta-Dental-PPO-1000-100-80-50-50-SG-2026-OR.pdf).)

Carriers disagree on whether the extra visit is free. One states a price: "An additional cleaning at a $40 copay is available to expecting mothers and members with diabetes." ([*BCBSAZ BlueDental 2026 Employer Group Product Guide*](https://edge.sitecorecloud.io/bluecross-6f8ea2ea/media/project/bcbs-az/azblue/data/media/files/employers/specialty/bluedental-product-guide.pdf).) Another adds the extra without changing the pooling, so the uplift raises a shared ceiling rather than creating a separate benefit: "Enhanced Service: Additional prophylaxis (cleaning) per year, traditional or gingival. Allowable Maximum: Up to 4 cleanings annually, using any combination of codes D1110, D4346 and D4910" ([*UnitedHealthcare Dental FEDVIP Official Plan Brochure 2025*](https://www.opm.gov/healthcare-insurance/healthcare/plan-information/plans/pdf/2025/brochures/02AP-15.pdf)).

Cigna's chronic-condition programme is worth reading precisely: it adds "One additional cleaning" and "One additional evaluation", and on fluoride and sealants it removes the age limits while stating that "all other limitations (including frequency limitations) apply" ([*Cigna DPPO Dental Office Reference Guide*](https://static.cigna.com/assets/chcp/secure/pdf/resourceLibrary/dental/DPPODentalOfcRefGuide.pdf), Oral Health Integration Program footnotes).

## Why it depends on the plan

The uplift is an option a group buys, so two patients with the same diagnosis at one carrier can get different answers. The window differs too. One document states the pregnancy window outright — extra benefits run for "nine months of pregnancy plus three months post-delivery" — and adds that the extras are cross-checked against periodontal maintenance rather than stacked on it ([*Delta Dental of Arkansas Evidence Based Dentistry Guidelines*](https://www.deltadentalar.com/docs/default-source/smiles-provider-resources/ddar-evidence-based-2022.pdf)). The same guidelines say the uplifts do not stack: a patient with a qualifying condition who also has periodontal disease is covered under the periodontal benefit, not both.

Enrolment differs as well. One plan enrols members automatically off a processed claim — "Patients are automatically enrolled … with the periodontal disease indicator when they have a claim for a periodontal procedure (surgical or non-surgical) processed" ([*Evidence-Based Integrated Care Plan (EBICP) Enhanced Dental Benefits Guide for Providers*](https://www4.deltadentalwi.com/assets/docs/EBICP%20for%20Dentists%202014.pdf)) — where another requires the member to register first.

And the extra cleanings are not free money: one state company notes that the "Costs of the additional cleanings/fluoride count toward the plan's annual maximum."

## What to do

- Ask for the benefit by name — the medically integrated, enhanced or wellness cleaning — rather than asking whether a third cleaning is covered. The second question is often answered no when the first is yes.
- Find out which evidence this plan wants before the appointment: a physician's statement, the diagnosis on the claim, a pocket chart, or registration. They are not interchangeable.
- Check whether the extra visit is covered or sold at a copay, and quote it accordingly.
- Ask whether the extra cleanings sit inside a pooled hygiene count with periodontal maintenance. On many plans the uplift raises the shared ceiling rather than adding a benefit.
- Warn the patient that the extra visits usually still draw on the annual maximum, and check the remaining maximum before scheduling a third and fourth.
